PLEASE REMEMBER TO SEND A PACKED LUNCH IF YOUR CHILD IS BOOKED IN FOR A FULL DAY

By submitting this form you confirm that:

• You wish to make a booking for our Holiday Club as detailed above.
• You understand the club may not operate if there is insufficient demand.
• You agree to be billed and to pay in advance for your booking in accordance with our current prices.
• Bookings are made in accordance with our Admissions and Fees Policy.
• If your child(ren) is/are not registered with us, your booking will not be accepted until you have filled out a Registration Form for each child.

Understanding the importance of vigilance regarding sickness is a keystone of preventing transmission to other children, staff and parents.

You should not bring your child into the club if:
•your child is experiencing any symptoms such as cough, shortness of breath or difficulty breathing, fever, muscle pain, headache, sore throat, or new loss of taste or smell.
•your child has tested positive for Coronavirus.
•your child has been in contact with anyone in your household who has Coronavirus symptoms.
•your child is not capable of following the club rules relating to protective measures, such as washing their hands regularly, coughing and sneezing into a tissue and disposing of it safely, and not touching other children. (If your child is unwilling or unable to follow these rules they may not be allowed to attend future sessions).

Cancellation Policy

Please note, if we do not receive notice of cancellation or changes by the Monday of the week before the session(s) to be cancelled/changed, payment in full for the respective sessions will still be chargeable

Our Holiday Club is open to any primary school age child from any school.

Please note that if your child is not fully toilet trained, we do not have facilities available or have staff ratios necessary to attend to children who have toilet accidents. Parents will always be called to attend in such instances and we therefore ask this be consider prior to making any bookings. Our recommendation is to avoid making bookings if you child has had any accidents in the previous half term.
